To ensure your VA benefits are processed quickly and securely, please read the instructions below before submitting your documents. Submissions that don’t follow these guidelines may result in delays.
All documents are to be submitted to vmaforms@ua.edu from your student “crimson.ua.edu” email address. Emails from outside the UA system will not be encrypted and therefore not accepted by the VMA Office.
The items listed below are required to process your benefits. Failure to complete the items listed below will result in processing delays of VA payments.

New Students
Submit all documents simultaneously to vmaforms@ua.edu. The VMA Office will not accept individual documents.
Certificate of Eligibility (COE)
The COE is proof that the student is eligible for VA Benefits. To receive a COE, apply for your VA education benefits.
You may submit a confirmation of your application as a COE placeholder for up to one semester.

DegreeWorks Printout
You can find this within MyBama. The printout should include a list of required classes for your specific degree.
All Students
These documents are required of all new and returning students, including Graduate and Distance students.
Advising Worksheet
Use the advising worksheet form within DocuSign.
Concise Schedule
Attach a copy of your concise student schedule from MyBama to the DocuSign.
DD Form 214 (DD214)
If you are a veteran, please submit a copy of your DD214.
Change of Major
If your major changed after you had already submitted an Advising Worksheet, you will need to submit a new Advising Worksheet.
Change of Major Form
If your major has changed, please fill out a Change of Major Form (PDF).

DegreeWorks
You can find this within MyBama. The printout should include a list of required classes for your specific degree.
Add/Drop
If you add or drop a class after submitting your Advising Worksheet, you will need to complete a new Advising Worksheet. This new worksheet needs to be signed by your advisor. Please also attach a copy of your Concise Schedule to the Advising Worksheet you submit to your advisor.
